Here are some common example templates that can be used in the various services provided as follows:

Generally

When requesting information from the ATO, a separate form is required for each entity. An entity includes an individual, partnership, trust, company or bodies corporate.

The 'Individual' form can be used for individuals.

The 'Other' ATO form is used for all other entities.

If you choose not to use any of these forms you can prepare a letter of authority which needs to include:
